Filing A Nursing Home Abuse Report
It is important to include as many details as possible when writing a Connecticut nursing home abuse report. There needs to be enough information in order to spark an investigation into your claim.
Make sure that the report is in writing (not just a verbal report) and that it is dated. It is critical that you keep copies of the report in case the original is lost or tampered with. When writing the report, be sure to include the following:
- Who: Include the names, ages and addresses of any victims; the name of the facility where the abuse took place; All individuals charged with the care of any victims; and a detailed description of the individual you believe has committed the abuse.
- What: Describe as accurately as possible the nature of the abuse, including signs of physical abuse and an estimation of how long you believe the abuse has been occurring.
- Where: Record where you believe the abuse to have taken place; for example, the resident’s room, in the dining hall, etc.
- When: As best as possible, account for the times and dates of all instances of abuse.
